Hi, all! I’m more than 6 months post-OHS for a bicuspid aortic valve replacement and generally doing well. However, I’m still having some sleep issues. Almost every night, I wake around 4 am and struggle to get back to sleep. I’m not having a pounding heart or racing thoughts. I’m just awake. I toss and turn and have a bit of trouble getting physically comfortable, but mostly I’m feel wakeful. It usually takes- me an hour or more to get back to sleep. Is anyone else experiencing this so long after surgery?

I use my adjustable bed to change position mid night, and also two pillows on either side of my arms to get more supported. (FYI, I was awake at exactly 4 AM too last night if that makes you feel better).

Richard Munson Don’t feel alone. I was up at 4 am for 2 straight days. Shadow standing on the bed woofing in my face is a wake up call that can give my mitral valve a little start. After a trip outside, getting back to sleep can be tough. It beats cleaning the rug though.

In the earlier days post surgery I was exhausted all the time, so will fall asleep quickly, but then wake up from discomfort, and then fall back asleep pretty quickly once I adjust myself and settle back in. Now, almost 4 months out, I could make physical adjustments half asleep, so I’m not experiencing the constant disruption, but if I do wake up, it’s my mind that is keeping me up. It’s not like there is something particular on my mind, but I am wide awake. I read that it’s natural to wake up a couple of times in the middle of night, that it’s part of a normal sleep cycle, so when it happens I go through an abbreviated pre-bed routine, and usually I can fall back asleep.

Okay, so this might sound odd, but… Since my surgery I’ve been smelling a phantom smell. When I’m sitting in my recliner in the family room, I sometimes smell smoke, like cigarette smoke. To my knowledge, no one has smoked in my house since we moved in years ago, and no one else in my family smells it. I’m assuming this will go away in time, but it’s weird. The staff assured me there wasn't any reason for even a third hand smoke presence. But then last night I smelled it here at home (newly built house, no one smokes...)! Our systems sure are disrupted! Just curious, where you ever a smoker? I quit in 1979. Maybe there is some deep de-toxing going on.

Anyone else feel they hit a wall during recovery? I’m 4 weeks postop after OHS to replace a bicuspid aortic valve. I’ve been doing well, feeling a little bit better more or less every day. But the past few days I feel like I’m going backwards. I’m weepy and sad and achy and just generally tired and unmotivated. I recently stopped taking all Rx pain meds, so I suppose that could be a factor. I was cleared for driving yesterday and I’ll be starting cardiac rehab soon, so maybe that will make a difference. Just wondering if anyone else is having a similar experience?

Rita Savelis After all of you've been through physically, you often have to later deal with an emotional recovery. It's hard. You've been through trauma and your psyche is reacting. Cardiac rehab can be helpful because there are others there who have been through a similar experience. There is a lot to process. I started to feel down about 5 months after double valve replacement. It can help to be vocal about how you feel, perhaps talk to a therapist. Be honest with your doctors. Your reaction is common although many don't talk about it. Everyone is trying so hard to get back to "normal" but it takes time and normal is now different. Take care. Take it easy.

Rita (above ) gives good advice about speaking with someone about your feelings. Things will improve with time, but those feelings are difficult in the meantime. Totally understand what you are saying! Recovery was an emotional rollercoaster at times.

Raj K Hi - 2+ weeks since discharge. I was on Oxycodene for about 10+ days post-discharge…one pill every 12 hours or to manage “breakthrough” pain, which I did 9am/9pm…there was only one day where I got the timing wrong (waited a little later in the AM until after PT, instead of sticking to my regular schedule) and that was a problem just for the day, as I just could not get ahead of the pain. As of last week, I’m off Oxy, and the docs guided me that within 12-18 hours post last dose of Oxy, I would be cleared to drive (which I’ve been able to do over the last few days). In addition to Oxy on dismissal, I was (and am now) on 2 x 500mg Tylenol - although I’m now dosing this 1 x 500mg every 4+ hours or so, or to manage the pain. So call it 7-10 days of prescription pain meds post surgery in conjunction with Tylenol, and have now stepped down to Tylenol alone. The pain was really around the incision at first (more to do with how I moved and how it would “pull” at the edges), and now that has dissipated. Now the pain is more muscular (right side shoulder/lats - broadly around where the incision was made) and definitely spikes up towards the end of the day. I have found the Tylenol to help - as well as daily stretching etc. Being vertical vs. prone also seemed to help me a lot too.

(Disclaimer-I used 2 of them when I needed a root canal 6 months after my heart surgery). Muscle pain and fatigue were my biggest complaints post-op. I honestly didn’t have much pain in recovery, and was mostly taking Tylenol even after the first 5 days. Everyone is different, and i am betting those Tramadol you have will be enough.

Everyone is different, though, as is their tolerance to pain. I just felt tight in my chest and a bit uncomfortable. I never really had severe pain.

But, they also gave me Gabapentin for nerve pain(incision) that I did take for probably 10-14 days after surgery. I didn't really feel any incision pain using that. Most of my pain was neck and shoulder. I also used heat and cold alternating and icy hot seemed to really help numb shoulder and neck pain early after discharge.

I’ve been using the heating function in my recliner and just bought some Voltaren to use as a topical, so I’m hoping that will help. It just seems that I’ve been given much more pain medication in the past for far more minor situations than I’m getting for OHS.

Raj K Jennifer - one more thought for the pain management…I went to our longtime massage therapist yesterday and she did wonders for me on the right back/shoulder (other side from where the incision was) as well as neck and shoulders…all that stuff was just wrenched and tight post surgery - she had me sit/kneel on the massage chair (vs. down on the table) with lots of padding and pillows for my chest and it did wonders…between that and the heat and using a pressure point roller thing, it helps a lot

Allen Carkner I took oxycodone for a couple days but tapered off quickly and would take half doses. Mostly tylenol after discharge at day 5 post-op. The shoulder pain was terrible immediately post op but eventually the sternum became the main source of discomfort. Everyone is different and heals different. It felt like they were surprised by my pain sometimes in the ICU. Someone told me younger people can be more sensitive to pain because the nerves are actually more sensitive. Not sure how that holds up but it made me feel better at the time. Feel better Jennifer.

Well, I made it out alive! No kidding that was the first thought I had when I woke up. “Well, that’s cool, I survived!” This is not an experience I’m eager to repeat, but I had surgery Monday and came home on Friday feeling not horrible, so I think that’s a win. Some thoughts on my experience…
- I wasn’t nearly as nervous the night before surgery as I thought I’d be. I ate pizza and watched a movie at home with the family, then actually got some decent sleep.
- The morning of the surgery was fine. I checked in early as instructed and sat in pre-op for a bit before going into surgery. The OR was cold and bright, as expected, but not too scary. The next thing I remember was waking up.
- I woke up out of anesthesia in record time. They told me I was doing great. I was not doing great. Sometimes all the positivity can be annoying! The pain from the actual surgery site was nothing in comparison to the pain I had in my head, neck, arms and shoulders. Being splayed out on the table and the muscular pain and tightness that causes can be really bad.
- And then there’s the breathing tube. That the worst. It’s terribly unnatural and scary to have a tube down your throat breathing for you. It took many hours before I was breathing well enough on my own to have it removed. That first night in ICU was honestly horrible. Not being able to communicate was so terribly frustrating. I tried to type out messages, but I was shaking so badly post-anesthesia that I really couldn’t use a phone. I recommend having a notebook handy to scribble on. That would have been easier. My husband and the nurse thought I needed more blankets because I was shaking so hard. They kept piling them on. I was actually burning up. I found myself becoming almost angry with them for not understanding. Have a plan in place for communicating with your family during this time.
- On the plus side, when I was finally able to have the tube removed, it was a quick process and only a bit painful for a moment. And what a huge relief! And I got ice chips! Nice cold chewy ice chips!
- Another frustration was the back and forth nature of recovery. I’d feel pretty well in the morning, then really bad again in the afternoon or evening. I had to remind myself that recovery isn’t a straight line. It’s two steps forward, one step back, but eventually, you’ll get there.

And I don't remember pain, just intense discomfort and complete exhaustion. My breathing tube was out before I woke up so I'm sorry to hear about yours. Take care. Take it easy. Also had the back and shoulder pain, tube down my throat for 20-30 mins (definitely didn't like that either). Each week you will notice a big difference. I didn't notice day to day but definitely could see weekly improvement. Take it slow and steady. Listen to your body when it comes to rest and don't push too hard.

I just find that it’s good to keep in mind that you’re going through a major surgery and some of it is going to suck. A lot. And that’s OK. To me, it’s about managing expectations. If you expect it to be fairly easy and it’s really hard, that can be tough to process.
I can also recognize what a miracle of modern medicine it is that four days postop, I’m home relaxing in my recliner, in minimal pain, just waiting for my family to finish cooking my dinner. ☺️

My PT person had me stand up so straight - I thought it would hurt but it was just uncomfortable. The process of straightening up help my core muscles remember what they are supposed to do.
My other pro-tip for post op is make sure you have a good travel pillow so you can sleep in your recliner/chair/couch and not have a sore neck. It takes quite a bit of energy to hold your head up, and the support from the travel pillow will help you get better rest & healing.

Like others, I did not remember anything about my surgery, not even what the OR looked like, and if I was awake when they removed the breathing tube, I have no recollection of it at all. Aside from my sternotomy incision, I had pain only in my shoulder blades, more so on the left. The nurses gave me hot packs to put around my neck and shoulder blades in the back, and they helped tremendously. That pain subsided before I left the hospital - but I was there a week.

It's not weird to be upset, it's completely normal. What a letdown when you were ready to go on Friday! Now you have to make it through another weekend waiting for surgery. And you will. nicole taylor Hi Jennifer, I completely understand your frustration. The anticipation leading up to procedure is so consuming. I was terrified for my operation and I just wanted to get it over with! Try to take it easy this weekend, make sure you are well prepared for post op, get some good nutrition and know that you will soon be on the road to recovery and living a full and healthy life. Susan Lynn Jennifer - I'm sorry you have to wait until Monday. You are totally right about being consumed with the thought of surgery every minute. Been there. We try to remind ourselves, whenever rescheduling occurs (and it does with frequency as Rita said), that we're fortunate to be heart warriors who aren't emergencies. Rich Butela My surgeon recommended to me to get the job done first thing Monday and be home by Friday afternoon. He said better staffing during the week vs the weekend. So maybe the delay is a good thing even though the waiting is hard.

Sue…I think my menopause issues are done. I hope. I do get pretty warm sometimes during the night, though.
