At 8.5 months after my surgery, I’m finally starting to have brief moments when my thumbs and first two finders (both hands) feel normal.
Until now, those six fingers have been numb and tingly all the time.
Melinda Donahue Hi John !! Great news, any trembling and did anyone give you a diagnosis?
John Cook Melinda, it’s been a while. How are you?
My nerves were pinched because of positioning during surgery. Apparently it happens to 5 - 10% of the time. I also have a numb patch under my chin that makes shaving feel weird.
I haven’t had trembling, just fingers “asleep”. But between the numbness and the swelling from my arthritis, my dexterity has been shot and I can’t do anything by touch. ... Read more

They said nerves are slow to heal and it could take a year or it might never fully recover. So I celebrate any little improvement I get.

Jeffrey Sweatlock That is great news and very encouraging. Have you been doing any special exercises, therapy, or steroids (like a medrol dose pack or injections at the wrist like for carpal tunnel)? An orthopedic hand surgeon did a few steroid injections at the joints in my fingers & base of the thumb. For me, it helped my arthritis & dexterity. How is your running progressing?
John Cook For me, since the numbness wasn’t severe, it’s just been something I’ve endured. I was more obsessive with my ability to exercise and dealing with edema.
The only injection I’ve had is on a trigger-finger knuckle that is related to my arthritis, not this numbness. The trigger-finger predated my surgery.
I haven’t done any physical therapy or special exercises for my fingers. The loss of movement I have is because of swelling related to my arthritis and predated my surgery. I might be switching up my arthritis meds in March to see if something different will help my hands more. ... Read more

I will be going to see an orthopedic surgeon at the end of March for my trigger finger. I will ask him about my other fingers, but more for getting range of motion back, not sensation.
I did have one package of Medrol after my surgery but it wasn’t for the numbness. I honestly don’t know what it was for. It was just one of the many pills they sent me home with.
My running hasn’t been progressing lately and that’s been my own fault. I was a slacker in December and January and put on a few holiday pounds. I’m trying to get back in the habit and eating better. Thankfully, I get 1.5 to 5 miles of walking at work though.
I will be purchasing a new pair of running shoes this month so I can be more adventurous with my trail running. It was part of my plan for last year that got derailed by my heart surgery.

Maureen Hoganson Hi John -- Glad you are improving! I had a Ross Procedure June 2nd at Vanderbilt after replacing my native BAV 11 1/2 years ago. I feel like I did well overall with the procedure, but because of compressed nerves in my neck, my thumb and two fingers would go numb on both hands usually at night sleeping. I have done a few sesssions of PT since my surgery last summer and my physiotherapist does think it was partly from the position I was in during surgery and just being "rearranged" again in my sternum, which as you know also affects your ribs and thoracic region. This time, instead of just sternal wires, they placed Longitudinal Sternal Stabilization plates (with wires). I think my system (muscles, nerves, bones) are still getting used to the new arrangement -- but my physiotherapist says she thinks the plates have me in a better position than the wires alone did, and I am continuing to try and heal and keep up with my PT exercises and exercise overall. Hope you continue to improve! It's hard to sort through some of the resulting symptoms from open heart surgery, so glad you posted this - especially since you had a Ross as well!

Melinda Donahue Hi.! I had carpal and cubital tunnel surgery both elbows and wrists before surgery and also 5 trigger surgery releases. I went thru the whole journey of steroid shots in fingers and wrist and nothing worked. Surgery helped but after ohs it worsened and the numbness returned and a bit of of trembling began. Went to neurologist and he checked for PD which I do not have. Sent me for more PT and it is helping. In my situation much has to do with the cervical spine and limited movement and weird sleeping positions causes some issues. Starts cardio rehab so out there doing something each day in addition to my own exercise. Hopefully a little better each day. You all help a lot. Thanks John your comments are always helpful ❤️

John Cook I don’t know why under my chin on the right side is numb. I didn’t discover it until I shaved for the first time because the port for my juggler took a long to heal. It was a couple of months before I noticed it.
My suspicion has always been that it was the port that damaged the nerve. But that’s probably just me displacing how much I hated having it.

Scott Mills John. I have had very low iron post surgery even though I’m on iron supplements. I was also on 1000mg of naproxen / day until recently, and am now wondering if that is / was contributing to it?
John Cook Yes, naproxen can contribute to low iron anemia. It can cause stomach irritation and blood loss. That’s why they also gave me Pepcid. However, Pepcid inhibits iron absorption because it reduces stomach acid.
To help iron absorption, you want to take your iron supplement with something high in Vitamin C & acidic, such as orange juice. If you take anything else that contributes to your anemia, wait at least an hour before you take that.
I usually use apple juice instead of orange juice. I have an orchard nearby that I can buy a gallon of fresh pressed juice (not concentrate) and it is so delicious. It’s acidic and between it and my multivitamin, I’m getting about the same amount of Vitamin C as orange juice. ... Read more

John Cook None of my neuropathy existed before my surgery. I think the fingers are because the median nerve was pinched in my neck when they positioned me for surgery. It’s not an uncommon problem post-surgery but it’s usually the ulnar nerve (the other fingers) instead. Basically, my fingers feel like they are ‘asleep’ any time I grip something. They said it could take up to a year before I know if it’s permanent or not. It was bad for the first three weeks when the tingling was more persistent and I was getting back on my arthritis medication. It was difficult to grip a glass of water, especially if it had condensation and was slippery.
I didn’t notice under my chin until the first time I shaved after surgery (2ish months). I put that off for a long time because the jugular port they put in my neck was slow healing. I wonder if putting the port in the vein damaged the nerve.

David, it’s more about my health finally being stable than the doctors. This odyssey started that April 2024 when I decided to get a new GP because I wasn’t getting good enough care from the island clinic. I went in for blood pressure medicine and discovered I had a heart issue. She put me on lisinopril, which kicked me off Naproxen, & triggered a massive PsA flare that took another year to diagnose. I’m still dealing with problems in my hands from that PsA flare.
My heart is finally stable and might improve a little over the next 8 months. My PsA is finally stable, is slowly improving, & I get another medication this month. I’m optimistic for a more normal 2026.

Yumiko Ishida My iron level was “critically low” at one point. Can’t remember how many months after surgery. It’s apparently common post surgery, but enough time had passed at that point, so my PCP referred me to a hematologist. It took a while to see the hematologist, so I got an iron infusion from my naturopath. By the time I saw the hematologist my numbers were still low but on an upward trend, so the hematologist did not think I needed additional infusion. If you want to avoid endoscopy, getting a referral to a hematologist for close analysis if your blood maybe helpful. There are a lot things that could contribute, not just blood loss, but other nutrients like C, B12, folate, etc.
I had a lot of bloodwork done today, 9 different panels. My red blood cell, hemoglobin, & related stuff is all off. But my kidney function, B12, folate, and iron all look good. Of course, I’ve been on iron & folate supplements. I did look back to when I went on methotrexate in April and my first bad anemia blood test was in May. My surgery was in June. I guess it could just be the methotrexate and I need an alternative for my arthritis. I’m hopeful the doctor reaches the same conclusion next week.

Rose Madura I played about nine months or so after my surgery. I did a little par three. My chest muscles were a little sore afterwards but it felt good to play.
It went as well as expected: 56 for 9 holes. I haven’t played much for 20 years. My last round was in 2021 or 2022.
I took a couple swings at apples in my yard before I went to the driving range and hit a bucket of balls before playing 9.
Between the stiffness of PsA and surgery, I’ve lost so much distance off my clubs. However, my hits are a lot straighter because I can’t overswing anymore.
My chest is doing fine but my back will be hurting for a day or two.
John Cook Thanks. Most days I’m good about my salt. Some days I still have difficulty.
My last dose of diuretic was 5 days ago and I’m down 1.5lbs since then. The last time I tried this I was up 9lbs in the same amount of time and my feet turned into footballs.
1.5lbs fluctuation in my weight from day to day was normal for me pre-op. So I’m going to take this as a very good sign.
I did a 3.7 mile hike/run yesterday. At Week 13, I finally feel like I can do everything I could do pre-op, I’m just operating at 75% speed when exercising.

Week 10 post-op update: I think I finally figured out how to get ahead of the edema and stay ahead of it. I’ve been gaining and losing the same 2 to 8lbs of water every week since I was discharged from the hospital with the use of diuretics on various cycle regimens.
I started wearing 20-30mmHg rated compression socks and I’ve managed to squeeze 5lbs of water out of my feet and keep it off for a week.
The bad news this week is I discovered I can’t reduce my carvedilol any further than a 25mg dose 2x a day unless I increase another BP medication to offset it. I’m checking with the doctor to see if I can try offsetting the reduction with an increase in losartan.

Robert Miller 5lb of water out of your feet? I am a little bit skeptical about this. 2.2lb of water is a cube of 1dm x 1dm x 1dm or about 4" x 4" x 4". Since you mention 5lb that 4" x 4" x 4" = 2.2lb would per per foot. I think you are losing these 5lb over your whole body. Having said that, do you really need to be on the "water pill" 10 weeks post op? It might also help reducing salt intake if you don't already do that. This should help with water retention and blood pressure. There are also other medications that can help with blood pressure and potentially heart rate and/or arrhythmias if you have that.

I went into surgery at 242lbs, went up to around 258lbs after surgery, and they made me drop down to 231lbs before they moved me out of ICU. Dropping all that water was the worst 3-days of my life. I don’t really remember the other 2-days.

Since discharge, when I’m not on diuretics, my weight will jump up to 240lbs and my feet are so swollen, you can’t see my ankles. Depending on my diuretic regime I can get down to 238, 235, or 231lbs, but it always jumps back up to 240lbs within 3 days of going off the diuretic.
Let me tell you, swinging from 240lbs to 231lbs and back to 240lbs over 7-days really sucks and I did it 3 weeks in a row.
With the addition of compression socks a week ago, I’ve managed to get down to my current 234lbs in 10 days with very little creeping back up on off-diuretic days and I have ankles again.
Also, I’m on so many meds for it, my BP is under 120 regardless of how much salt I eat or how much water I’m retaining.

Robert Miller Well, I guess for you it makes sense to be on the water pill. Is the pulse of under 120 when you do things? If it's that high just sitting at the computer and such then you probably want to get it further down. It's my understanding that ideally your pulse shouldn't be more than about 90 when at rest.

John Cook BP = blood pressure. My surgeon wants it always under 120 unless I’m exercising. If I creep over it at rest, I’m supposed to let my doctors know so they can increase my drugs and knock it back down. It’s part of his protocol for Ross patients.
Most days, my resting pulse is between 50 & 55. Part of it is the drugs and part of it is regaining some fitness. I’m starting formal walk/run interval training this week.

Robert Miller A pulse of 50 to 55 is excellent. I was there in my teens. Don't really know later on until I had my surgeries a couple years ago. I wish I was there. It's my understanding that if your heart beats at 90 or less most of the time you are fine. Systolic blood pressure below 120 is good. Also look at diastolic because that is the pressure that is constantly applied. But you are probably just fine.

4 updates for this week (week 8 of recovery) 1. I finally got my doc to let me cut my carvedilol. I wanted to cut to 3/4ths but he let me cut it in half. I feel better and my blood pressure is still within my normal range since I left the hospital. I especially don’t feel like I’m breathing through a straw with an elephant on my chest when I’m walking up hills. 2. I figured out the source of my rib pain: stretching - doing things with my arm fully extend even with no weight on it. Like when I reach to turn off my CPAP, turn on a lamp, grab my phone off the charger, etc. It hasn’t been overdoing it on lifting or my short jogging intervals. 3. My edema is weird and makes no sense to me. One day, my feet will be loose in my sandals. The next day they will barely fit. The next day they will fit perfect. It doesn’t seem to correlate with sodium intake, water intake, weight, exercise, or medication. Sometimes I think I can actually watch them grow bigger and smaller over the course of a day. Any insights would be appreciated. 4. The slight anemia I had pre-op got worse post-op, and finally got so bad this last week they put me on iron supplements. I do think the iron is helping my energy level.

Then they changed me to on Monday, Wednesday, & Friday. I’m on my second week of it. I lose 3lbs over the first 3 days. Then I’m steady for 2 days. Then I gain it back and restart at the +8lbs.
My feet swelling is not directly related to my weight. They can get bigger while my overall water weight goes down for the day.
I’ve been better about my salt for the last month. I’m using MyFitnessPal to help me track it. I’m averaging about 1700mg/day.
I’m not tracking my water but I’m drinking it excessively either. I’m pretty sure I’m under 2 liters/day.

Robert Miller My cardiologist discouraged me from rehab. He told me that they slow you down and just do simple stuff and then increase it and then claim this was all because of the rehab. I guess it depends a bit on the individual but the point is, just stay active. You have to be careful the first 3 months with doing heavy work. But go for your daily walks, do breathing exercises for a week or so after surgery, use something like an elliptical (less stress on your chest than a treadmill), etc. The first few weeks are a pain but it's all for the better...

John Cook Robert, what I liked about it was: - They asked me what I had been doing post-surgery. I told them I was walking 1.5 - 2.5 miles at a time up and down hills and I had started very short downhill interval jogs. - They asked me what I wanted to get out of it. I said I was running 5Ks at 4.5 mph and an RPE of 4 pre-surgery and I wanted to get back there by the end of year. I said that I also wanted to schedule a 10K run in October of next year. - They pushed my heart rate harder than I had been willing/able to push it myself. It was the first time I got my pulse above 110 excluding bouts of v-tach in the hospital. - After evaluating what I was able to do during rehab and how my blood pressure reacted, they said, “You are okay to keep doing what you’ve been doing.”

Thomas Brusstar Good news, John. The most important thing for me was the point you make, namely "They pushed my heart rate harder than I had been willing/able to push it myself." I also found the personalities of and encouragement from the cardio nurses at the gym fun and energizing!

It's not a long-term habit you want to get into, but your body is working overtime to heal and that takes energy. Don't be so hard on yourself. You'll eat more when you're ready.

My suspicion has always been that it was the port that damaged the nerve. But that’s probably just me displacing how much I hated having it.
