I’m lucky that I have a hospital in my state that can do my surgery. My sister in law just had bypass at Mt. Sinai. She loved the facility.
Let your surgeon advise you on what type - I always ask ‘if I was your mom, what would you do for her’? They really have the answers. (Not that you shouldn’t be involved, but don’t get married to one idea either - there might be considerations you don’t fully know about.)

Hi Robert. I’ve been through the process of choosing between multiple hospitals. I would recommend meeting with the surgeon and cardiologist who would take care of you at each place, and see if there is a consensus about exactly what to do. If you have agreement, then you can pick a place with the best outcomes, statistics or the most experienced surgeon or however, you want to choose. But if there is a difference in recommendation, then I suggest you think that through carefully to see which procedure you want and decide if there is a place where you would be better off getting it either because they do more or because they are advocating the approach you want to take. Good luck with this. I know it’s scary and not easy. Over the past decade I’ve been through TAVR and OHS, both out of state. In my case, I went from Boston to Cleveland, and I would do it again. There were extenuating circumstances.

Fidel Martínez Ruiz Hello Robert: I´ve read your story and if I were you, I wouldn´t rule out open surgery if were necessary, not waiting too long. I´m convinced that the important is to have a skilled surgeon, whether in TAVR or in O.S. I don´t know about it but perhaps surgeons can treat both valves in an only open single surgery. I wish you to choose the best way.

Robert Teichman I know open heart is probably the best solution, and yes they could deal with both valves in one operation. However, I am very scared of open heart, and the associated pain of breaking the breast bone. Also, the mortality rate of repairing aorta and mitral at the same time is significantly worse than just repairing/replacing the Aortic valve.
