All is good! The Heart has settled down nicely to low 60s bpm and showing nice smooth regular curves on my heart rate meter. I have been walking early every morning for 3 to 4 miles and have found this an excellent way to start the day, and means that I clock up about 8 to 10 miles a day. The other plus is that I have rediscovered the beautiful city of Amsterdam and I have it all to myself just after dawn. So for week 3, I can only report it’s only getting better and better.
One thing I did learn was to buy a digital blood pressure meter for about 80 USD and charting my results each day. Just helps to chart the progress. So I am off for a walk!

So my first week at home has been great. Walks around the neighbourhood, light lunches at the local restaurant, and plenty of resting and napping all supported by my daughter and lovely wife.
Then bang day 13 and I woke up in the middle of the night with atrial fibrillation and my heart rhythm bouncing around. I relax do my breathing excercise and walk a lit. Still no good so I call my medical team and they request an ECG, thankfully my local GP obliged asap and the team upon receiving the results call me in ASAP.
The good news is it’s not because of fluid build up as before. Apparently AFib happens to 40% of patients after open heart surgery. So they change my meds by increasing the blood thinners and changing the beta blockers and send me home. Let’s see how it goes. Not too concerned as I expect it’s just my heart trying to adjust to all the new parts.

The body is a miraculous thing. Tuesday evening in the operating theatre for a second time, and then Friday evening home sweet home.
Being home with my wife and daughter is wonderful a further boost on the road to recovery. Now about that road ? How much to walk and when?
My iPhone step counter showed me doing 1,7 miles on Day 6 / 7 in hospital as I did my laps around the ward and practiced the stairs. The question now is that I am home what distance of daily walks should I target? So far I have ambled around the neighbourhood but I am ready for more. Any advice?

Day 3 started well and so was I doing a little walk and then my heart rate went crazy and one the great guys here called Stefan came and asked me If i was alright. Apparently all the alarms were going off. After that long surgery a lot of fluid had trapped itself around my heart and my heart rhythm was all over the place at +135 BPM. The drugs were not working and I weakened fast. My doc explained its like running a marathon for 2 days. Day 3 evening I was not good at all. Morning Day 4 I was bad. Day 4 lunchtime my surgeon advised to go in again ASAP, full Anaesthesia and drain out that fluid, and reboot my heart with an electric shock. Well as I was lying there pre op round 2 on day 4 I was as weak as I could be and a whole lot less positive than even facing the original 8 hour surgery.
Now the good news, that evening I was already improving, that night i listened to some inspirational talks and was just happy to have got over that bump, and today day 5 has been the best yet. I am back on that recovery road. Looking forward to tomorrrow when the last of the drains come out.

I will meet the surgeon soon to found out what the final way in was but in the end and a surgical tool as introduced to remove 450ml of fluid in my pericardium sack, allowing my heart to be restarted safely with a controlled “electric “ reset of the heart. My heart is now purring in low 70 and The hardest part was going through the op process again and going back a number a number of days. The best thing was the taste of the ice lolly on the way back to the MC unit.

It was also shocked. They also did a pericardial window. No doubt you fee much better now. I know I did. Best wishes for continued recovery.

The good news is that I have a surgery date (June 15), the not so great news is that the surgery is more complex than we could have anticipated.
Not only a valve replacement but a bentall procedure to replace all of the ascending aorta and most of the aortic arch. After 2 CT scans the dilation of my aorta was much greater and much higher up than first recorded and this means the operation will be much bigger and all my hopes of minimal invasive etc are out of the window. It’s going to be a big one. The two surgeons performing the operation say it will be an estimated 7 hr surgery.
My treatment :
Valve replacement with an Edwards Inspiris Resilia valve via a bentall procedure including a Hemi-Arch Repair of aortic arch aneurysm.
Nonetheless I am keeping positive and getting myself ready for the big day. It’s good to read the positive recovery case studies on this website and its giving me some positive energy. Thanks amongst others Steven Perrotta!

I’m not discrediting the CT place but it was definitely a different kind of medical place...like a house literally vs a hospital or lab.

My cardio team warned me that the measurement of the echo scan is normally smaller than the CT scan. My earlier echo focused on the aortic root and heart, the CT scans picked up the higher ascending aorta and arch more clearly. My aortic root was 53mm on the Echo and the CT. The issue is my ascending aorta near the aortic arch is dilated to 59mm.

I did have complications but I'm very symptomatic. I had complete faith in my doctors and their team of professionals. Michael Eichhorn Hi Tobias, I am 3 weeks 4 days post Bentall Procedure with Hemi Arch. My surgeon told me that the surgery was complicated. Turned out to only be 3 1/2 hours. He told me wife after that the valve Edwards Bovine 27mm fit perfectly and things went smooth. I am feeling great walking 3-4 miles a day. Today I watched my 13 year old play two baseball games. Finally had my echo and recieved the bad news that my ascending aorta is dilated to 53mm. Cardiologist thinks I shall need to undergo a Bentall procedure. I was hoping on a tissue valve and if possible the New inspiris resilia valve via minimal invasive surgery. My cardiologists says that will not be possible and now it will most likely be open heart via a Bentall procedure. Started researching and read that new revised guidelines indicate that only if >55mm replacement is neccesary. Also that in Australia a mini bentall procedure has been used with a 7cm incision. Would welcome any advice and guidance.

Marie Myers I had a full sternotomy for my AVR, root replacement and ascending aneurysm replacement. My aneurysm was 48 mm, but because of my petite size, it was recommended that I get it done "sooner than later". Guidelines recommend rescecting an aneurysm at smaller sizes if it is associated with a bicuspid aortic valve. So depending on your history and physical size, the guidelines may differ.
As far as minimal incision size, you must take into account the surgeons's training and experience with that type of surgery. You will find different techniques at different places. But these are all good questions to ask at your consultations .

I am 6ft 3 and 200 lbs but even so my aorta is certainly dilated. I am just hoping that it is stable. The problem is I have limited information from my cardiologist as on average my 2 consultations to date have lasted 5 and 8 minutes, and I am told to wait until I meet a surgeon in about a months time. I had come to terms with a minimal invasive valve replacement and was ready for that. This latest news has set me back. As I live in Europe I will most certainly need to look outside of the Netherlands to find a competent surgeon in mini bentall procedures.

Felix Cherny Tobias! I really don’t think they can do a Bentall procedure + aortal valve replacement with a minimally invasive technique. Even if they say they can, think about it. When they open you up fully - they get a full and unobstructed view. Cosmetics vs quality of surgery - what would you chose? A scar will be healed and unless you are a male model nobody pays attention to that. Have you finalized your decision on the valve type? I really think it’s too risky at your age to go with the tissue. Read some stories here, people have to get re-operated in less than 5 years. There is no scientific evidence whatsoever that at your age tissue valve will last even 15 years with a high degree of certainty. And trust me, repeat surgery won’t be fun!

Don’t worry about the incision size, this should be the last thing on your mind. Just find a surgeon that you trust and go with him. If you are not comfortable with the quality of healthcare in the Netherlands, I recommend IKEM Hospital in Prague for you! You will be satisfied and the staff speaks English here, and your EU wife insurance should cover most of the cost. Good luck!

I was also recently diagnosed with a melanoma skin cancer on my face and so I am not so keen to undergo CT scans too often due to the radiation. My cardiologist has requested CT scan on mah 7th yet I don’t have a surgery date yet. I fear I will need another CT scan before surgery. Is that correct? My doctors have so far failed to answer that question. Also is MRI an option for an initial check and then a CT scan before surgery? Thanks in advance for any advice.

The only CT scan I had "with contrast" was in early December 2017 to confirm clear arteries, and so my surgeon recommended for me to schedule the date for anytime within 12 months. Soon after the CT scan I had a TEE test which revealed that my Mitral Valve status was actually severe instead of moderate, so I moved up the surgery, and had my mitral valve repaired in March 2018. So- at least in my case, almost 4 months passed and that CT scan was still relevant.

I had a heart catherization in Jan. 2018. I am to have surgery on June 5, 2018 and at that time I will have a TEE. As far as I know, I will not be having a CT scan prior to surgery.
Marie Myers I had 2 CT scans prior to my surgery, because the doc needed to see if my aneurysm was enlarging.
