A question for Adam Pick & members; I recently had a very successful Mitral valve Replacement surgery on April 15th whereas I purposely selected the Edwards Lifesciences "Mitris Resilia" Bovine Tissue valve due it's better technology anti-calcification properties just like the Aortic "Inspiris Resilia" Bovine Tissue valve has. ,,,,At the age of 64, now I'm situated for the future to utilize the valve-in-valve procedure utilizing the Edwards Lifesciences "Sapien 3" technology. ,,,,,,,I was directed at Jefferson Einstein(Montgomery) to take Coumadin for the 1st (3) month period due to the Mitral valve being a low flow type of valve where some blood can sometimes get stagnant versus the Aortic valve I had replaced back in 2016 being categorized as high flow. ,,,,,,,,,Does anyone know if the caffeine of having (1) coffee in the morning can affect the effectiveness of keeping the INR therapeutic range from 2.0 to 3.5....???

I have since 18 months an Edwards Magna mitral valve- size 31, the former model to your Mitris Resilia, this last one being supposed made of more advanced anticalcification tech and tested for many years in aortic position. The same as you, the surgeon proposed me "valve in valve" when the valve wears out. In my case, I donΒ΄t feel any alteration on my levels of INR, taking into account that the surgeon close my left atrium appendage and I follow up now taking edoxabΓ‘n. Rita Savelis Hi Barry. I have taken coumadin for 10 years for 2 mechanical valves and I drink as much or as little coffee as I like. INR will go up and down no matter what you eat or drink. You can live your regular life, eat and drink as normal and then adjust your meds around that. However, it can be especially hard during your first months on coumadin (it depends on each person) to get into and stay in your therapeutic range. It takes time. And INR can do funny things so one can never be sure what it is that is changing it. After my first year I stopped worrying and just carried on. I test once a month and don't think much about it. Never had a problem. I adjust my own meds but find that they almost always end up at the same dose. All this to say - go ahead and have a coffee or two coffees.

Susan Lynn Barry - I was on blood thinners for three months after my mitral valve repair surgery and I wasn't asked to give up my morning coffee, but I did need to be consistent with my diet to ensure I was medicated appropriately. However, I did develop PACS and PVCS and caffeine can make a difference with those conditions. My electrophysiologist said he wouldn't ask me to give up one cup a day though.
This past year, however, I switched to decaf. There are much better choices than the old Sanka we drank decades ago. My cardiologist was also able to cut my dosage of metoprolol succinate in half without my PACS and PVCS increasing in frequency. My hope is to continue to reduce the dosage.

Would I have been eligible for the Ross Procedure back in 2016 given that I had Upper chest mantle radiation therapy for the Early stages of Hodgkin's disease(2A) back in 1988. I'm a 37 year "Survivor" & doing great thus far!!! ,,,,I had purposely chosen the Edwards Lifesciences 3300TFX Bovine Tissue valve due its impressive track record of performance & longevity whereas many people already achieved 15-17yrs minimum with it. ,,,,,,,,I'm currently at 8.5yrs & the valve is still in excellent shape. ,,,,,,Just still curious if the Ross procedure could have been done back then because I wasn't aware of this procedure until after my very successful surgery on July 5th,2016. ,,,,,,,,,,,,Barry

I powered thru the overall surgery with relative ease. ,,,I had missed the release & approval of the Inspiris Resilia Bovine Tissue valve by less than a year or else I would have selected it due to it's even better anti-calcification properties.

Barry Kanick Hello Adam Pick,,,,,,,,,,I will keep this in mind!!! ,,,,I've been engaged with Dr Raymond Singer again & had all my testing done & was about to have surgery on June 5th(My Birthday) with Dr Raymond Singer at Jefferson Einstein(Montgomery). He decided to play it even more safer & cancelled the surgery. ,,,Based on testing Results, He now wants me to get a 2nd opinion & has aligned me with Dr Samir Kapadia at the Cleveland Clinic to go out & talk to him to see if the "TMVR" procedure can even be done instead of another OHS. ,,,Dr Singer already had talked to him on my behalf. My election fraction is still at 58/60% & the main construction of my Coronary arteries are just fine. Just a little stenosis on a few small branches. ,,,,Currently, I am asymptomatic with no Issues at all. Just have to deal with the Mitral Valve itself relevant to calcium deposits which is now categorized as severe stenosis. ,,,,,,,,,,,Dr Gillinov actually was a 2nd opinion of mine back in 2016 for the Aortic Valve because of the same exact issue but I still chose Dr Raymond Singer due to his phenomenal Track Record of now 8000 (+) hugely successful Heart Surgeries with great outcomes. ,,,It was in the vicinity of 7500 back in 2016.

Susan Lynn Barry - I asked my surgeon at my one-month appointment how long I was on bypass and he replied about an hour and a half. With the pre-op and post-op surgical activities, I don't know the exact total time. My husband thinks it was about three hours. I also worried about being on the pump for an extended period of time for this entry. The cannula is placed in the femoral artery and I did know that prolonged surgical time could present a problem. But, I was honestly more worried about what extended pump time would do to my brain rather than my leg. I chose a high-volume, experienced minimally-invasive mitral valve surgeon for that reason. I initially wanted full robotic until I found out that it could take significantly longer.
During my consultation, I also asked about early extubation - I didn't want to be on the ventilator for a long time, either. I even reminded the anesthesiologist on the morning of my surgery that I chose Dr. Goldman and his team because of their approaches to quick tube removal. I remember talking to my sister and husband by 3:30 p.m. without so much as a sore throat.

I will keep all this in mind. ,,,My Aortic valve replacement back in 2016 was estimated to be around 2-1/2hrs overall additionally. ,,,,Ironically enough, I had kept this article about Dr Raymond Singer ever since 2018/2019 with the intention that I may use it as leverage some day. And here we are!!! I had showed it to him back in early May during an appointement at Jefferson Einstein(Montgomery) & he hugely got a kick out of seeing it.

Invest in yourself by purchasing & thoroughly reading Adam's Book titled "A Patient's Guide to Heart Valve Surgery". It was the best $29.95 I had ever spent ever!!! ,,,You'll get educated & develop an understanding relevant to everything as to what's about to happen to you every step of the way. ,,,,,,#2) Do your homework & research regarding the past history of the Thoracic Surgeon, his track record of success & efficiency that you are hiring to do the job. Do not let any Cardiologist just randomly select any Heart Surgeon for you!!! ,,,,That being said, the longevity of the surgical team he's been working with also hugely matters too. They are usually all in "sync" like a finely tuned Ferrari because they've worked together for 15_20_25+ years. ,,,,#3) Based on your age category,study your valve selection to ultimately make your choice & discuss it with your Cardiologist or Thoracic Surgeon. It's the information age & there is a ton of information online relevant to this. 4.) Rent an electric Recliner for the downstairs which will slowly stand you up vertically. During the 1st 2-3 weeks & possibly more, you simply don't want to be pushing off getting out of a conventional bed with your arms & be putting any unnecessary strain on your upper chest area where your incision is. 5.) After the surgery is over & you leave the Hospital, have a family member(s) or friend(s) stay with you 24/7 for the initial 2-3 weeks as a just in case. It will give you total peace of mind knowing someone is there. But if any of them are & were previously sick around the vicinity of helping you, tell them to stay away. ,,,,You certainly don't need any additional unexpected issue(s) to develop why you are recovering!!! ,,,,.,,,You simply have to ask yourself the what if's!!! ,,,Example_ What if I wake up at 2am with atrial fibrillation/accelerated heart beat or any other related side issue(s) that could possibly happen??? ,,,,,,,Mark my words, I did my sizeable research in a span of 3-4 months, acquired (3) 2nd opinions from some of the best Thoracic Surgeons around the Country & only then committed to a surgical date because I was 110% totally prepared "knowledge" wise. ,,,If you make the time to study the overall process relevant to what I'm actually saying here, you'll be totally prepared & most likely anxiety & stress free knowing your in terrific hands overall. ,,,,,,,I myself actually walked out of the hospital (3) days later after the surgical date on only 500mg of Extra strength Tylenol & barely even needed that. Barry Kanick Additionally,,,,,,,,I hired Thoracic Surgeon Dr Raymond Singer at the Lehigh Valley Hospital(LVHN) to replace my aortic valve. He has already achieved 7000 + Surgeries with excellent results & in my opinion is clearly one of the best Thoracic Surgeons on the East Coast. ,,,My valve of choice was the Edwards Lifesciences Magna 3300TFX Bovine tissue valve because I had just turned 56 years & one month old right before the surgical date of June 5th,2016. The valve has a heck of a longevity record of 10+ years & more plus I'm an active guy who hikes the Continental Divide in Colorado.For me personally, there was no way in hell I was gonna put up with the continual maintenance of a mechanical valve. The Cleveland Clinic_Penn Medicine & LVHN all agreed with my decision & selection because of my age & stated that TAVR will most likely be even moreso perfected down the road by the time I may need another. ,,,,Better days ahead!!!

Would also stress on #4 to make sure your friends and family coming into your home understand exposure to illness is a risk. I.e. Strep. Had that experience with a close family member the day I got home, who did not understand why it was a big deal. Have backups and people who will value your health as much as you do.

I got it from Rent-A Center store. I simply just covered it with a premium bed sheet in combination with a nice comfortable pillow. Additionally, a small TV tray next to the Recliner with my few medications, your spirometer & cell phone worked out just beautifully. My brother slept upstairs!!! ,,,To Kelly Stoll,,,,,,thanks for reminding me. I actually did tell anyone if they were recently sick to absolutely stay away. Barry Kanick Additionally,,,,,,,,I hired Thoracic Surgeon Dr Raymond Singer at the Lehigh Valley Hospital(LVHN) to replace my aortic valve. He has already achieved 7000 + Surgeries & is one of the best on the East Coast. ,,,My valve of choice was the Edwards Lifesciences Magna 3300TFX Bovine tissue valve because I just turned 56 years old one month before the surgical date of June 5th,2016. The valve has a heck of a longevity record of 10+ years & more plus as an active guy who hikes the Continental Divide in Colorado, there was no way in hell I was gonna put up with the continual maintenance of a mechanical valve. The Cleveland Clinic_Penn Medicine & LVHN all agreed with my selection because of my age & stated that TAVR will be even more perfected down the road by the time I may need another. ,,,,Better days ahead!!!

I did not rely on the surgeon of choice by any of the 3 cardiologists I received consults from. In fact- I learned that as soon as OHS is in your future- go right to the surgical consults and get several opinions from high volume surgeons in your specific heart valve issue! By doing my homework and getting an exceptional surgeon I had peace of mind knowing I had done everything possible to be ready to recover fully from OHS! And this community- yβall have inspired me and given me the experienced answers I needed in every practical and unique aspect of OHS!

Today marks exactly (5) weeks that I had my Aortic Valve replaced with an Edwards Lifesciences Pericardial Magna Bovine Tissue Valve(Model #3300TFX) & I definitely feel terrific. I get up each day, have a little breakfast & then walk my Road course that I setup in my development for approx 2-1/2 miles. I then do the same in the early evening when the cool temperatures exist. ,,,,,,,,For those having upcoming August & September surgery(s),,,,try not beat yourself up beforehand & please don't compare yourself to other peoples outcomes & then anticipate the same issue(s). ,,,,,,Everyone definitely has different experiences & levels of patience!!! ,,,,,Back to work on August 15th to an easy Design type job sitting behind computers for Mack Truck Inc.( 6 weeks off was just right for me) ,,,,,,Hell, I don't even consider my job as work anymore!!! ,,,,,,,,,,,Barry

The purchase of Adam Pick's book was the best $29.95 I have ever spent & invested on my own personal Healthcare!!! It provided me with the leverage of knowing every little detail relevant to the overall "Aortic Valve" Replacement procedure before & after. I had a total awareness as to what to expect & was totally prepared on all fronts. This is what I hugely give credit to when tackling the overall Hospital stay with ease & all the sequential things they had to do from Day1 to Day 5. I knew exactly what they were gonna do & why before they actually did it. No anxiety,,,no spikes in blood pressure,,,& no loss of sleep leading up to the Surgery date of July 5th. One of the single biggest things I extracted from Adam's book is how he got family & friends to stay with him for the 1st (14) days when discharged home after the initial Surgical week in the Hospital. Yesterday was my 1 week Post Op date & I am writing this update so I can possibly hugely help the next person who has this Surgery in the future. My surgery was hugely successful on July 5th & the next day I walked all the way down to the end of Ward to my new private room in the Transitional Open Heart Unit @ the Lehigh Valley Hospital(LVHN.Org). On the 3rd day post op(July 8th),,they even contemplated already releasing me but I stayed Day #4 just to play it safe.,,,,During the (4) days after the surgery I was in "zero" pain & sometimes only a slight discomfort when I intermittently coughed but that's what the "Heart Pillow" is for. ,,,,I walked away from the Hospital on only 500mg of extra Strength Tylenol. I never really had the need to accept Oxycontin or Oxycodone. Being that I am hugely an active person & hike the Continental Divide in Colorado all of September each year. ,,,,I had chosen the Edward LifeSciences Pericardial Bovine Magna Tissue Valve Model #3300TFX. This is the most current latest generation of Bovine tissue valves available these days. I additionally chose this specific tissue valve with the notion that the TAVR procedure is perfected even moreso by the time I'm 70 yrs old(just 14 years from now). My current tissue valve has an estimated longevity timeframe of 10-15yrs from now therefore they can just plop another new tissue valve right into place within the Valve I just recieved via the femoral artery in my upper leg. ,,,,I also had chosen Dr Raymond Singer here at the Lehigh Valley Health Network due to having one of the best successful track records here in the State of Pennsylvania. He had performed approximately 6000+ Cardiac surguries of which an estimated 3000+ involved "Heart Valve" surgeries.,,,,,I would most definitely highly recommend him to absolutely anyone!!! Dr Raymond Singer actually took the photograph above whereas I documented & shared with him how I researched & mapped out several of the highly touted Surgeons from around the country demonstrating their extensive Accomplished Track Records.That all being said, I would hugely recommend to anyone to purchase Adam Pick's book(The Patient's guide to Heart Valve Surgery). ,,,,,,I read & studied that book twice before surgery & it provided me with a total awareness of what to expect when it comes to absolutely every relevant detail of the Surgery before & after. ,,,,,,,,I was totally prepared!!! An educated guess tells me that your trying to duplicate the inclination of the electric hospital bed for comfort. That being said,wouldn't pulling the typical large lever on the side of a recliner aggravate your sternum area somewhat??? ,,,,Unless your always having someone do it for you ,,,,,,,,Barry

I didn't realize it & just made use of the one I have at home work. But sleeping in the recliner is def the way to go. I tried the foam wedges for my bed & hated them. I'm heading into week 3 post-op & finally sleeping in my own bed on an incline with propped pillows that seem to be working for me (which didn't the first two weeks)

Steven A Husted yes you are right pulling on that lever on the side of a recliner would hurt you and may cause more damage to your sternum that is why most patients on here recomend a electric recliner with a push of a button it goes up and down and they are very comfortable most people use them for a short period of time until there sternum heals but i use mine all the time and i am 20 months out i cant sleep on my stomach any more because it hurts my sternum so i sleep in my electric recliner and it works ok for me

I felt they were more comfortable than my hospital bed and was able to sleep a full fours on my first night home in this recliner, It also helped with my cough and since my husband had to go back to work fairly quick, it made it easier for me to get out of my chair (it literally puts you in a standing position) plus I feel it encouraged me to walk which is super important the few first weeks when you are still pretty uncomfortable. It's actually only $24.99 per week to rent. ,,,,Then I went to an Ashley Furniture Homestore here in Pennsylvania & they actually had the exact same one on the floor so I actually tried it out. It will actually stand you up close to 70 degrees of standing perfectly upright. ,,,,Works just nicely & comfortable!!! ,,,,,Just out of curiousity, I then went to a Raymour & Flanagan Furniture store nearby to check out typical pricing on other electric models & they would typically run somewhere between $600 to $900 depending on your preferred brandname. ,,,,,,,Thanks to everyone for your valued input!!! Since December of 2015, I have recently acquired (2) 2nd opinions from Dr Gillinov @ the Cleveland Clinic & Dr Joseph Bavaria @ Penn Medicine near Philadelphia that is actually just a 1hr 10 min drive south of me.Both suggest to have aortic valve replacement surgery soon(next 3 months) due to my Av area dipping below 1.0 to .7 for the very 1st time late last year since we discovered a heart murmur in 2010. It was then 0.6 on January 29th,2016.,,,,I am currently asymptomatic & have no issues whatsoever.,,,,I still feel like a million bucks & still swim after work each day. That being said,,,the leading culprit for the calcification on the aortic valve seems to be due to the radiation I had back in 1988 for the early stages of Hodgkins disease.The New England Journal of Medicine even states a 30yr latency period for possible valvular disease. I'm currently near that at year 28. ***As an outpatient for the Hodgkins Disease in 1988,,,the doctors were marveled how I actually blew thru the treatment like it was a common cold!!! ,,,,I am now currently debating with myself on selecting the Cleveland Clinic(6hrs away) or Penn Medicine(1hr_10min away). ,,,,,,I do realize Cleveland Clinic has been ranked 1st in cardiology for last 15 years & Penn Medicine is around being 7th. ,,,,,,,Any thoughts/input from anyone on this issue would be greatly appreciated!!! ,,,,,,,,,,Barry(56yrs old come June 5th)

Cassie LaFramboise I just went through surgery and had this same struggle. I didn't know to go with a prestigious hospital or go with my gut and stay in Boise and go to my surgeon for my situation. Even though my situation should have been handled at the mayo clinic I had confidence in my surgeon and I chose him because it was where my support system was for my recovery and just felt more comfortable. It's completely up to you but if I had to choose again between traveling or going to the top, it didn't matter to me. Do what's most comfortable and helpful for you

Kate Watson I had surgery with Dr Bavaria 11 years ago. I went to him because he was highly recommended by my cardiologist. Just 2 weeks ago, I had surgery at Swedish in Seattle and it was a completely different experience. It was a long time ago that I went to Upenn but I felt they did not prepare me or my family for anything that would happen along the way whereas Swedish told us step by step what to expect. I hate to say this on a public forum, but Dr Bavaria told me he put in the biggest tissue valve he could fit into me in the hopes that it would last longer. That decision caused my latest surgery to be complicated. My surgeon said it looked like a valve was stuffed into a spot that was too small and that the tissue grew all throughout the wall of my heart, so they had to scrape it off causing them to have to replace my aortic root and for me to lose a lot of blood. He said he never saw anything like it. My surgeon though would not say anything bad about Dr Bavaria though and says he is highly respected. I saw three surgeons before picking mine this time and I knew he was the right choice. I was so glad I chose him because he was confident and knew how to deal with this complication. My opinion is to go for the best you can! Hope that doesn't scare you and I'm not

Anne Walpole I am also asymptomatic with my aortic stenosis and my valve area decreased from 1.0 to 0.6. My cardiologist said I can continue to work out and don't need surgery yet. She just wants me to see her in 6 months instead of a 1year. I am wondering if I should get a second opinion, because I have read that when the valve area is at or below 0.6, it's considered "critical" and some patients might need surgery even if they have no symptoms. I decided on my own to stop working out on the treadmill even though she said I didn't have to stop that. I do still lift weights up to 12 pounds and go on walks.

I vacation in Boulder,,Summit County & the Aspen/Snowmass area practically every year myself & want to retire out there in 2-3 years. I'm not overly worried about getting my aortic valve replaced in the next 3 months because I'm already hugely informed before I commit to a specific surgery date. Barry Kanick Anne Walpole,,,,,,,,,Thank you!!! In July of 2015, my Av area was at 0.7 and then on January 29th,2016, my Av area was 0.6. I was told that 0.7 is usually labeled as a "severe" category & 0.6 or below gets labeled as a "critical" category. When your Av area is below 1.0,,,,,,I've read numerous times where a 6 month checkup is moreso appropriate than a yearly checkup. ,,,,,,,Barry

Erin Hannum Hi Barry! I had my aortic valve replaced and mitral valve repaired by Dr. Gillinov in February. I felt very fortunate to have him because I was referred by a friend and Dr. Gillinov watched my progress over an 18 month time span. I took his opinion very seriously.
Every aspect of my care at the CC was wonderful! I felt very comfortable with Dr. Gillinov, as well as the staff members I met prior to and the day of. They were good to me as they are to all patients. The Cleveland Clinic claims "World Class Care" and in my experience, they lived up to it. Also, the follow up and attention I received after surgery and at home was very good.
Best of luck although you don't need it. All will be well and you will make the choice you feel most comfortable with.

Marilyn M Hi Barry. I do feel lucky to be only an hour from Cleveland Clinic. In Feb. My av was .67. I mentioned Dr. Gillinov to my cardiologist. She said some surgeons are booked far in advance. I was originally looking at the end of May, but I had symptoms and felt I needed to get in sooner.
I researched and found Dr. Douglas Johnston who uses sternal plates instead of wires. (There is a video about this on cc website) But when I met with him he said because I only had aortic valve to be replaced he would do a mini thorocotomy through the ribs. I also asked if he could put in a bigger valve for tavr. It's good to do research and get opinions. Best of luck to you which ever you choose!

Barry Kanick ***Thanks Marilyn*** I currently have no symptoms whatsoever & currently feel like a million bucks these days. ,,,What do you mean when you say "asked to put in a bigger valve for TAVR"??? ,,,,,,,TAVR is only designated for much older people right now who can't withstand traditional open heart surgery. I'm actually set on getting a Bovine tissue valve for now due to being a very active guy thus knowing that the TAVR Technology gets even better 10-15 years from now when the tissue valve supposedly wears out. ,,,,,,,,,Barry

Rita Savelis I had radiation for Hodgkin's at age 15 and 38 years later I had to have both my aortic and my mitral valves replaced and my tricuspid valve repaired. Although ultrasounds showed my aortic valve calcifying, I had about 5 or 6 asymptomatic years before surgery (at age 53) last year. Sometimes doctors will leave it up to a patient as to when to have that surgery. Often there are no symptoms even if an ultrasound looks bad. One may be asked to have surgery with no symptoms or a surgeon may wait until you have those symptoms. It's really different for each patient. I suggest having a surgeon who is familiar with operating on post-Hodgkin's patients as the state of your valves will reflect that radiation and how surgery should done.

,,,,,,Yes a large percentage of our patients are asymptomatic and much like yourself and very active.
Your AVA can be under 1.0 and you may not have the symptoms, that is what makes it hardβ¦β¦β¦.. to have heart surgery and feel normal going into it.

Cathleen Weed Hi Barry! I'm 44 and just had TAVR last August. I was approved for the procedure at Cedars Sinai LA due to my 3 previous AVR's with traditional OHS. I had my previous calcified, stenotic bovine valve from 10 years prior replaced with another bovine tissue valve again this time. I too am VERY active and was fairly asymptomatic going in. I have had my aortic valve replaced four times and I can tell you that a stenotic failing valve feels VERY different from a regurgitating failing valve. The stenotic valve will fool you into thinking you're okay. The regurg valve, well you feel like you're dying. Trust the numbers though, not just your symptoms. I almost waited too long. My valve was closed to the point of a tiny straw...not good. Get it over with is my advice! A new chapter awaits ππ and TAVR will be perfect for you next time around!

I recently had a very successful Mitral valve Replacement surgery on April 15th whereas I purposely selected the Edwards Lifesciences "Mitris Resilia" Bovine Tissue valve due it's better technology anti-calcification properties just like the Aortic "Inspiris Resilia" Bovine Tissue valve has. ,,,,At the age of 64, now I'm situated for the future to utilize the valve-in-valve procedure utilizing the Edwards Lifesciences "Sapien 3" technology. ,,,,,,,I was directed at Jefferson Einstein(Montgomery) to take Coumadin for the 1st (3) month period due to the Mitral valve being a low flow type of valve where some blood can sometimes get stagnant versus the Aortic valve I had replaced back in 2016 being categorized as high flow. ,,,,,,,,,Does anyone know if the caffeine of having (1) coffee in the morning can affect the effectiveness of keeping the INR therapeutic range from 2.0 to 3.5....???
This past year, however, I switched to decaf. There are much better choices than the old Sanka we drank decades ago.
My cardiologist was also able to cut my dosage of metoprolol succinate in half without my PACS and PVCS increasing in frequency. My hope is to continue to reduce the dosage.
