Hi Chris- I was told to read your journal after I posted about my situation. Looks like you and I are in the minority of people who go through the agony of mitral valve repair, only to have it fail. I have been struggling with whether to have a redo or replacement. I have had 3 Dr.s looking at my TEE's and one said I need a replacement and one said he thinks it could be redone and the other doesn't think the problem is my leakage, since it doesn't look that bad. I have had all different types of test on my lungs, diaphragm, etc.... I still feel like I did before my surgery, ie- tired with the slightest bit of stair climbing, walking up hill or walking for any length of time over a minute or so. I am really frustrated and feel so let down. Wish I could go see your Dr., but as I live in California that's not feasible. I was referred to the best Heart Surgeon on the west coast though and he said my TEE doesn't really show that much leakage, but it's very hard to tell from the different angles. Just wanted to vent to someone who's been through this. Sometimes I just want to scream "why me?"

Now that you have had a second surgery I would like to hear what you think.I am 52, but our youngest is 11 and we home school.Worried! I'm sorry that it took a while for me to get back to you. Samantha and I have been away on vacation for the past week or so.
I see you've been getting some good advice from Deb Z. Very nice lady. She and I had surgery in Cleveland around the same time frame and actually only lived about 20 minutes away from each other at the time.
I will give you my thoughts on both of my surgeries but I encourage you to do your research and ask Dr. Gillinov a lot of questions.
My first surgery was in September of 2011. It was performed robotically and repaired my posterior leaflet and replaced some of the chordae. The repair was apparently successful as the post surgical echos showed only trace amounts of leakage. Initial pain and discomfort weren't as bad as I imagined they would be and were much better after my drain tube was removed. I was able to leave the hospital in 4 days and flew home. Sleeping was rough for a few weeks and I had some skin sensitivity around the incision site for a few months as I recall. Recovery time was pretty short. I was able to drive quickly as I was off pain meds in pretty short order. I attended cardiac rehab and seemed to be doing well. Then I had my three month echo and found out that I once again had a moderate to severe leak. I couldn't believe it.
After more tests and sending my results to Cleveland to get their thoughts, it was decided that I needed to have a re-operation performed. I scheduled it for June of 2012, nine months after the initial procedure.
This time the surgery was performed via a sternotomy due to scar tissue from the first surgery. I dreaded this procedure for fear of sternal pain and recovery issues. Truth be told, I believe there was less pain with the full sternotomy. Recovery time was certainly longer and there were more restrictions, but again, nowhere near as bad as I had expected. At my follow up echo last October, everything seemed to be working just fine and I'm feeling good.
Knowing what I do now, if I had to choose today between robotic and traditional (or recommend one to a family member) I would choose traditional. Since the time of my first surgery, I have met or conversed with at least 4 people who have had issues with their robotic procedure, requiring a second surgery be performed. We may be exceptions to the rule but that seems like too many cases to me. In addition, I believe the extra recovery time of the sternotomy is a fair price to pay for the surgeon to have a full clear view of everything going on.
I hope that doesn't scare or confuse you. That is just how I feel after my experience. I still liked Dr. Gillinov and thought Cleveland Clinic was a great place, but maybe the robotic procedure wasn't the best choice for me. I would encourage you to ask Dr G. questions about the robotic procedure. I asked quite a few before I had it done, but I would be interested in knowing how many robotic mitral valve repairs he did during 2010 and 2011 and how many of those had to have another repair done within a year or two (I know of at least 3). Also , how many were done traditionally and how many of those needed a re-op.

I was very interested in your story since I also had a robotic mitral valve repair done at the Cleveland Clinic by Dr. Gillinov on March 17,2011. The result was the same. Severe regurgitation returned within a month. I have not yet had a re-do. Obviously, Dr. Gillinov is a brilliant surgeon, but I am left wondering about the robotic failures.

I have also been diagnosed with anemia and see a specialist about that in December....all in all I am still alive...watching my New York Giants and Michigan State Spartans blow fourth quarter leads..

I am coping - started back to work on Wednesday - full time is stressful, especially with having to work cardiac rehab into my schedule. I may see about working 1/2 days until I get through rehab. I am exhausted after the exercise - concerned that my moderate leak is causing alot of fatigue - still upset that I even have a leak at all after going through major surgery....you know the feeling...
I will be going back to CC in November once I complete cardiac rehab for an echo stress test - not sure what that might reveal, just taking 1 day at a time.

Another HVJ blogger told me I should read your story. I just had MV Repair surgery at CCF on 5/29/12 and found out the results of my July echo that I have moderate to severe regurg and an EF of 40-45%! I am devastated by this news. Dr. Roselli at CCF did my repair and we are going to CCF on Friday to have a TEE and Echocardiogram done and meet with him later in the morning to talk about all of this. I am not sure how I feel about a re-repair as I am 59 - wondering if a replacement may be a safer option. There is so much to think about and it is all so very scary. Wondering if a reoperation could be done before the end of the year since I have met my max out of pocket with insurance for the year. You can read my journal to see what has transpired with me since surgery (seroma issue, etc.).
I would appreciate any guidance or advice you or Samantha could give me. Thanks so much!

I've been following your story and am sorry to hear about the possible post-surgery leak. Having gone through it, I know how devastating that news is.
Sam and I were wondering what type of procedure you had, minimally invasive, robotic, full sternotomy etc? It seems to us that some procedures are more prone to issues than others (robotic in particular).
I struggled with the re-repair vs. replace question. We talked at length with Dr. Gillinov about the options and decided that he would have to make the decision once he was into the operation. A re-repair to me was the ideal situation, but if there was ANY question that a re-repair would not outlast a replacement tissue valve, then I wanted him to go ahead and replace the valve. According to Dr. G, the previous repair was intact, and the problem area was a new issue. He said it was very localized so he felt the re-repair was the best option. I hope he was right.
As far as having a re-operation yet this year, I was told by Dr. Gillinov that ideally the heart needs 6 months to recover from an operation before they would want to do another surgery. Maybe sometime in December would be an option if necessary.

I\'m finding the second time around, within 6 moths of my first OHS a real challenge. I\'m so grateful they were able to repair instead of replace. I\'ve found that the emotional anxiety and generalized weakness to be the most depressing. I\'m on a short course of coumadin this time, and am convinced it causes generalized yuckiness all over my body.
