Best of health to the HVS community! I'm looking for a physician with expertise with senior (old) athletes with a bicuspid aortic valve. I'm a 65 year old cyclist with an untreated bicuspid valve looking for training guidance as the performance of my valve begins to deteriorate. I've been lucky in that my valve has been fairly stable since it's discovery at age 42. That stability has ended as my valve has developed increased stenosis and classification. I want to continue to cycle and test my limits, but not at risk doing long term harm to my heart. My current cardiologist is great, but has little experience with aging athletes. Can anyone make a physician recommendation? Thanks!

DeWayne Adamson Jim, I spent a lot of time researching surgeons. It is well known that the high volume hospitals have the best outcomes, so I would start there. My choice for surgeon is Dr. Eric Roselli at the Cleveland Clinic. I also spoke to Dr. Marc Gerdisch at Franscan Heart Valve center in Indianapolis. He is very good as well. Adam has numerous videos with both of them on this site.

Ted Niedziela Hi Jim, It seems like a
high percentage of us cyclists wind up with having heart issues! I had
AVR/AAgraft OHS about 2 years ago and now back riding again. Finished a 100km
gran fondo last week in about 3 hrs.
As far as cardiologists, I would search
out one you have high confidence in, especially that you will need
intervention/ follow ups/ testing etc. If you need surgery, use the list of
heart hospitals on this site. I had opinions with 3 different thoracic surgeons
and picked the one with most experience. Post surgery cardiac rehab is the time
you'd want to get with a with a cardiac therapist with a background in athletic
training to return to cycling form.

Ted Niedziela Hi Jim, I think the guidance a doctor would give on exercise levels depends on a number of factors (aortic dia., EF, BP, etc.) I was told exercise was ok and encouraged at a moderate level pre-surgery. I would experience paroxysmal AF due to my AV insufficiency when I exercised >70% max HR. This was the big motivator for me to have the surgery. No issues or restrictions now that I have the AVR and graft.
