I had a Mitral valve replacement at the end of January 2022. It was replaced with a On-X mechanical valve. Also had an ablation and closure of left atrial appendage at the same time. It has now been a year and my Warfarin/Coumadin levels are not stabilising. I did 30 INR tests last year and already 7 this year. My level are too low. For about 2 months last year they were fine at about 2.7-2.9. It has been below 2.0 this entire year with several increases in my Coumadin dosage. My diet is controlled with the minimum of green veggies and my meds are prescribed by my Cardiologist. I take my meds every day and no other meds or supplements. I'm so frustrated with this. What may be causing this problem? Are there any tests that may indicate what the issue may be?

Klara Čičić Dear Hennie, I was on warfarin for only 2 months but kind of had similar experience. First month everything was ok, INR in range... But afterwards, despite diet changes and increase in dosage to the maximum allowed, my INR just didn't wanna go up. Luckly I didn't have to take it anymore after 4 months so it wasn't problem for me. I would revise in detail all drugs and food and drinks you take that can interact with warfarin. If there is nothing to "blame", in some people, genetic changes in liver enzymes (ones that metabolize warfarin) can develop. That causes warfarin resistance. In that case, one is to try another vitamin K antagonist drug similar but not the same as warfarin (there are few, for example acenocoumarol...) Good luck

Hennie van Rensburg Thank you very much for the reply Klara. I will have to take warfarin for the rest of my life. I will speak to my doctor or cardiologist regarding taking another vitamin K antagonist drug. I provided my doctor with a list of what I eat and all the medication I take and he said it was fine. It is really frustrating that they cannot "fix" the problem with the low INR. The Cardiologist told me that I'm at risk for a stroke if it stays below 2.0.

I would revise in detail all drugs and food and drinks you take that can interact with warfarin.
