Hey! Saw your post about leaving your job! Good for you!! OHS changes a person, physically, and mentally! Good to see you pivoting to what makes you happy!! 👏👏 When I was a supervisor, years ago, I used to tell the guys, I was mentoring, that if they hated the job, then stop going to something every day that did not make them happy!
I believe that one of the priceless life lessons that OHS has to teach all of us, is that this life is short, and if we just spend it focusing on getting ahead, or money, or doing something we hate, then we need to stop, and turn towards what makes us, and our family, happy.
God teaches us that to gain the whole world, but lose our souls, is pointless.

I am actually making a HUGE change… I am leaving my job. My job is very physical, and it completely drains my energy, so when I get home I have no energy to play with my daughter, and no energy to get any real exercise in. Lately, my only days of exercise are on Wednesday’s when I go to physical therapy. So I am going to be leaving my job, getting a new exercise routine at home, dropping off and picking up my daughter from kindergarten, and I also am starting my own business working on cars. It’s risky, but my wife is so amazing and is a big reason we are able to make this happen. She is so supportive and thinks this will help in a huge way, and will get me back to my normal happy self.

My wife and daughters, my health, their health, everyone’s happiness in my home, spending more quality time with one another, all those small moments, are what matter.
Honestly, before surgery, I had this feeling that my surgery was going to change me, change something on how I viewed life and my priorities. I had my surgery and months went by and I’m thinking to myself… uhh when is this gonna happen? Well sure enough, almost 6 months later, the changes started to trickle in, the priorities shifted, and I feel as if I can see much clearer now..

Ya I think the people that have told me that and the things you read online is used as a general statement, for people that might not eat so healthy already, have to maybe watch what they eat and to monitor INR closely until they get it dialed in. Each persons idea of eating healthy is definitely different from one person to another. I am really glad tho to hear you are having a good experience with your mechanical valve :)

Andrew - I met with Dr. Chung at UW Heart Institute on Wednesday. She mentioned that I might be a candidate for the Ross Procedure with Dr. Burke. I am hoping to speak with regarding the procedure. From the patient recovery perspective, what are the differences in having a Ross versus just a simple mechanical valve? I understand that from an ongoing basis it does not require blood thinners, has a 15-20 year expected durability (although that would mean I might need something when I am 75-80, which probably isn't great) and people seem to have fewer overall problems - but is recovery significantly longer? Andrew Dial Hey Earl, glad you were able to meet with UW Heart Institute! So the main differences between mechanical and Ross. Well… mechanical is supposed to last a lifetime, so no surgery later in life, Which is huge, and in many cases what patients want. Downfall, is the blood thinners for life. And having to watch your diet and INR levels closely. I think most people adapt well… others Not so much. The Ross, the big advantage is not having to take those blood thinners and can eat pretty much whatever you want, live a perfectly normal lifestyle, etc. I am 33, and the Ross was a no-brainer for me… I’ve been told that at 20 years, there is a 20% chance of reintervention… so that puts me into my mid 50s for my next and hopefully last surgery. The Ross is highly technical as I’m sure you’ve read. I believe They both require a full sternotomy, so recovery is the same if I’m not mistaken. I think the main factors really for you are your age, if your pulmonary valve is able to be swapped over, and you are aware of the length of time you will potentially get from the Ross. The only other source I could find that had actual data going out to at least 20 years was from the German Ross registry which published a study last year of 2444 Ross patients. It showed similar but slightly better numbers when compared to the series at Baylor, reintervention in this series was 38.5% at 25 years.
One other thing to keep in mind is that since two valves were operated on, it means two valves will need intervention, and there's no guarantee they will fail at the same time. There's a link to Arnold Schwarzenegger's heart valve surgeries on this website, it's believed he had a Ross procedure in 1997, then a pulmonary valve replacement in 2018, then an aortic valve replacement in 2020, so three surgeries in 23 years. In addition reoperation on the pulmonary autograph (your new aortic valve) will likely be another open heart procedure, according to Dr. Stelzer of Mt. Sinai "Importantly, stenosis of the autograft has never yet been seen which makes valve-in-valve catheter-based therapy impossible for the failed autograft with current technology."
The Pulmonary Homograft can usually be replaced with a transcatheter valve but not always, in Schwarzenegger's case it ended up being another open heart, and based on what Dr. Stelzer said about the Pulmonary Autograph, that was also likely an open heart procedure.
From what I've gathered the recovery is similar to any other SAVR option. Operative mortality is also similar if you have the surgery at a center with a high volume of Ross operations.
Overall it seems like the Ross has some great advantages, but it comes with some real risks in relation to reoperations as well. Some surgeons will argue reoperations aren't a huge deal, others show increasing risks with every surgery, owed to scar tissue and such.

Greg Lewis If i could add a couple thoughts here: if you have to take blood thinners, its not just the meds that are at issue...your blood does not clot well. You then have to be careful what you are doing. I work where there is a high potential to get cut. And i work alone in remote areas, so not ideal. Also with a natural valve it can be monitored for its gradual decline. I dont believe a mechanical will have that. It will just stop, if it quits working. But when i was deciding, it was for sure something i considered, that i may not need another valve replacement, with mechanical. Also, can they do TAVR on mechanical? Insert a new valve into the old one? For me, it boiled down to the blood thinners, and the stroke risk, and the serious event of the mechanical getting clotted, and the gradual decline monitoring. But there are no guarantees here! As i have learned. So, i think you do what you feel is right for you, and then NEVER look back! Good luck!!!

From what I've read this isn't because the valve just stops, rather it's caused by paravalvular leak, pannus formation, or inadequate ACT leading to thrombosis.
Ultimately the best decision is the one you make, all the options have pros and cons.

I am older but also in very good shape and it took that long for my resting heart rate to drop to the low 70s. Still wasn't there at 12 months. That aside, I know the NW isn't like the NE in terms of cold, but seriously no heat?!! Is bringing a space heater in to much to ask? Though my guess is the space you work in is huge or you are sometimes outside. Are you getting a second opinion on this? Perhaps a stress echo could be done, because you would be running on a trend mill and that would test stress on your heart.

Klara Čičić There is this thing called reffered pain in your right (or both) shoulder which comes from phrenic nerve irritation...which can be due to many different reasons (it can occur after thoracic surgeries or if someone has gall stones, gall bladder inflammation or liver problems...or diaphragm spasm...truly many reasons). I hope your doctors can find out the cause with some more lab/imaging... I am struggling with pain in my shoulderblade, but it lasts since the surgery so I am not worried since it is probably due to nerve damage during thoracotomy. I must be patient. Keep us posted and hang in there! We are thinking of you.

But you know medicine, nothing is impossible. Tests will show. I bet they will do everything in their power to figure out the cause. Try not to google and panic... There are really number of issues that can cause your symptoms and good history taking, physical examination, labs, imaging...it will all help finding the cause.

There’s a pediatric/congenital guy here that would be doing along WITH the other surgeon, so the fact that there’s two both gives me some confidence, but also suggests

Hello Andrew, Remember you're really early in recovery. I'm 21 mos post-op from my Ross. For the first few mos I had occasional lightheadedness that caused me some uneasiness. Light headaches, too, that were difficult to predict and that I still get now. They're frequent, but short in duration and 2 for 10 on the pain scale. Rarely had headaches before surgery. Take your time with rehab and exercise. Stay hydrated. With time you'll think less and less about your heart and will regain confidence in its strength and resilience. That said, after surgery it's normal that you require your repaired heart to prove itself to you before you can relax your vigilance. That's where time and patience can help. All these words just to say I wish you well in your recovery. Tight lines, Andrew!

Hi again Andrew. I apologize for not mentioning this yesterday but if you experience anything that freaks you out again you can call 911. The department in the area that you live (Central Pierce) provides paramedic services and likely has a lot of capability to help you (heart monitor, O2 Sat., vital signs, etc. ). You can call and have them evaluate you. If it’s something like an arrhythmia then you will know asap. If nothing is going on you DO NOT have to go with the ambulance to the ER and can sign a release of liability or AMA (against medical advice) form and stay home. That’s what we are there for my friend and we run calls like this all the time. I’m sure your local guys/gals are outstanding and would be happy to help you. Just wanted to make sure you know that this option is available for you and that you’re not alone.

I got light headed at times if I had done too much . It ‘s still very early days . Try and pace yourself . You ‘re doing really well and you ‘ve had a lot of surgery . It ‘s hard to be a patient “ patient “ sometimes . It ‘s not a race to recover. listen to your body . It might be that your medication ? needs reviewing . Try not to worry , easier said than done , I know !

Hey Andrew, I saw your comment about having a lot of migraines after surgery. Starting a couple days after my surgery, I had 7 migraines in 7 days, it was awful. After that, it went back to my normal occurrence of migraines. I bet it has something to do with with being on the bypass machine and the toll heart surgery takes on your body. I've had two other surgeries in the past (not heart related) and never had that kind of response. For a couple decades I averaged 6-12 migraines a month, but then in the last year or so they have been very infrequent due to a nerve pain medication I take for my back. I would just give it a little time to rebalance, and deep breaths to get you through it. Of course, if you have any concerning symptoms, or it doesn't improve, you'll want to get it checked out.

Hi Andrew , Sensitivity to artificial sweeteners in food or drink can trigger migraines . Also chocolate , bright sunny days , lack of sleep . One of my son ‘s used to get really bad migraines . We looked at everything holistically and eventually found it it was artificial sweeteners that were causing them . An optometrist suggested sunglasses on bright days , so simple but it worked . He rarely gets migraines now , massive relief . I hope you have managed to sleep .

Andrew, when I woke up (consciously) from the surgery in the middle of the night into the next day, I was beyond weak and exhausted. Know that is normal, particularly when the sternum is cut.
Hardest part is over!!! You will be feeling so much better each day. Hang in there!

Andrew, Well according to my calendar you should be 1day out of surgery. Hope all went well. My 42 yo son had the Ross Procedure at U of Michigan on Feb 16, 2022 and is remarkable. He chose the Ross for basically the same reasons as you. He was in very little pain and home 4 days after surgery. Followed Dr orders to a T and can do everything except drill a long hard drive when golfing. He expects to get an OK on that in a few weeks.
His #1 piece of advice is to get a hand held shower head to be in control of the waters ' spray....when showering/bathing. Good luck you'll be up and around in no time and feeling just fine.
