Can't believe it's your turn already. I have been thinking about this day for over 20 years. Ever since I found out that I had a bicuspid aortic valve. I haven't had the need for surgery yet, but I feel as I am going through the prelims just reading all your posts. I feel I will be so much better prepared for when it's my time just having you there to talk with. Wish I had some advice for you. You seem to have a ton of people in you corner with lots of insight.
I know you will come through with flying colors. You are extremely well prepared. You have excelled in everything you have ever done and this will be no different. The big cry you talk about, I have had many of those just thinking about you. It's has taken me a while to write, because every time I start, I can't bring myself to finish. But I just wanted you to know that YOU have been in my thoughts everyday since I found out that you needed surgery. I look forward to coming to see you after your out in NYC. If there is anything you, Carol or the family need, don't hesitate. Wishing you all the best, talk to you next week.

Wow Jeff, busy weekend!! So...it's almost here. I'm crossing names off my list and so far everyone is scheduled for some time in March. I do predict though, that the list for April will begin soon. I'm really amazed at how many of us there are out there!
So...you sound really prepared. We have had a couple of curve balls thrown our way this week, but we will do this!!!
Thanks for all of your support throughout this ordeal and it has been an ordeal. I look forward to being able to say, "you know...even Mitch wasn't lying!", (sorry Mitch, couldn't resist). Waiting really was the hardest part.

Thank you for visiting my journal. You will be in good hands at Mount Sinai. Sounds like you have done all you can do, and are in the right frame of mind. Everyone deals with this in their own way, and who can question how they do, but in my case my acceptance brought me peace. I have been on both sides of the fence, watching a loved one go through surgery. To me it was easier being the patient. It sounds like your wife will have lots of support..that is good.I went through surgery and recovery with no complications but a fast heartbeat which was brought under control with medication. I will be in that band of angels praying for you. You are in great hands. God speed!

Hey Jeff, may you TREASURE this time with the constant reminders of your own mortality as life gleams much clearer in these times--what or whom is most important seems so easy to determine now and what/whom you value most becomes that which you want to be around the most. Sometimes the daily grind of life's details make this much more difficult to see. We pray for you and for your conversations-with your loved family and friends, as well as with God. These are great times to talk with Him because I think we're better able to listen for His answers. He promised in the Holy Scriptures of the Old Testament that if we seek him with our whole heart, we will find Him, and He will be there and be our God. Our family is here for you guys too! Can I chime in here about the survival odds and what you can do to get the best edge? It seems like a good place for me to get this down on "paper", I hope you don't mind my hijacking your guestbook for a bit.
1% doesn't sound like a lot, but we're talking your life. We've all faced this thought and it is, more than the surgery, what changes us - at least upstairs. Especially that last moment before we go under when we're not 100% sure we'll be back. Now that takes a special kind of courage!
The challenge is to tip the odds in your favor as much as you can. But of course, no guarantees. So what are the things you can do?
1. Have your surgery done at a high volume center of excellence by a highly experienced specialist team. This one choice reduces your risk by the largest factor. There are some high volume teams that do hundreds of surgeries a year that have zero loss per year. Avoid a "convenience" surgery at a community hospital.
2. Time your surgery as early as possible after meeting the criteria for surgery. By undergoing surgery while your heart is the healthiest you reduce your risk twice. First, you are stronger and more able to withstand the surgery, and secondly you avoid the pre-surgical risk of having an adverse cardiac event. Depending on the condition, once meeting the criteria for surgery, but opting not to have the surgery, the survival rate is about 95% per year. So a few months wait is as much risk as the surgery itself.
3. Choose repair and valve sparing options over outright replacement to the extent possible. This preserves anatomical features that enhance long-term heart health.
4. If you are looking at a replacement (aortic, primarily, but also virtually always as a mitral backup option), carefully consider the tradeoffs between life-long anticlotting therapy for artificial valves and eventual resurgery to replace a tissue valve. This will involve lifestyle issues.
5. All else equal, lean toward the approach (open heart, robotic, etc) that your team has the most experience and success with. You want them to have learned how to deal with every possible problem on those of us who went before.
6. If your valve surgery is expected to be challenging, consider full open heart (and a team that is expert in this) so there is a good view and lots of workspace to deal with contingencies. Also less time on bypass. Long time on bypass is a known risk factors.
7. If you are planning on minimally invasive robotic surgery, have a discussion with your surgeon about the added risk of stroke (about 3x higher) and ask what measures he or she implements to manage this risk.
8. If you are skinny, you may get a slight edge by adding some bulk prior to surgery since low body mass is a known surgical risk factor.
9. An afterload reducing medication may in some cases reduce cardiac remodeling prior to surgery, which can reduce your risk in 2 ways. Your heart will be stronger during surgery and recovery, and it reduces the likelihood of an adverse event prior.
10. While I haven't come across any studies on this one, probably if you have pre-existing conditions like diabetes, it helps to get them completely under control.
11. Enter the surgery well rested and not exhausted from anxiety and stress. This may involve some serious self-programming and even medication for some people.
12. Pick someone to be your advocate who will monitor and fight for the best possible care for you at the hospital. Everything from making sure your medication is right to being there with a clear mind to make the right decision on the spot if needed.
13. After surgery undertake a formal cardiac rehabilitation program. While it hasn't been tested for valve patients, this is known to improve long-term survival in bypass patients by 30%.
Taken together these various choices multiply together to reduce the risk not making it pretty significantly. Think of it as "making your own luck."
I'm not a doctor but did make a study of valve surgery risk reduction to be sure I'd have the best chance of being around 20+ years post-surgery. Facing my own mitral repair surgery, I followed each of these and everything turned out well. The odds were with me, of course, but I wanted them to be the best they could be.

Okay Jeff.... In my humble opinion, which really means nothing to anyone, especially my kids, I think it's time to stop listening to the well-meaning tales. You are going to be FINE. it's scary to be sure and your reactions have been the most normal I've ever seen you. However, I'll tell you what Sol told me right before he went in for his heart surgery.... "I've got the best of the best Drs., the best support team of anyone and the odds are greatly in my favor for success. If everything goes well GREAT! If not, you'll (meaning me) be crying, I'll never know the difference.". This may seem like a silly statement, but the surgeon said that he wished all his patients had that attitude. You've done all you can to prepare, now put your faith in others to do what they do best. I realize it's a feeling of losing control...of not being in charge of your destiny. Yes Jeff I did receive it, thank you. I also finally received the Peggy Huddleston book and CD. I was reading that in the ortho doc's exam room when I went to get my foot checked, had him look at the affirmation statements and he said "that is balony". Wow...won't be letting him operate on me any time soon. Anyway, between the 3 different imagery sessions, I may never come back to reality, lol.
Yes, my daughter is going with me to Cleveland. I wish we had thought to get someone else to go also to be with her, but everyone is either fighting health issues also or working. She's like her mom, though, actually better than her mom. She is my hero! She is also a licensed nurse, although she does not work as a nurse. But when she did, she worked on the surgical unit, so she will be a huge help. Just wish she could have some support while I am in surgery, ICU, etc.
So...wait we will, you for another week, me for two. Then relief!

Happy to read your update. You are ready and will do great!! My father had 5 open hearts, bypass and aorta replacement starting at age 39 and into his late, late 70's. He said key to recovery is positive attitude. He lived a long full life. I used his outlook to get me through my surgery and recovery. Great update, Jeff - it does seem that you are in a good place 1 week pre-op. You know I'm wishing you all the best.... A good attitude is at least 75 percent of everything when it comes to this kind of thing - and it looks like you've got that in spades!
By the way, I NEVER had that cry. But good for you that you could let it out. Hope you feel better after it.

I know you said how much of a positive person you are, and I am sure that helped with your recovery. I spoke with Dr. Stelzer yesterday and mentioned I spoke with you and he knew about the website and "Tin Man". He sure is sharp. :)

Hi Jeff,
Thanks for the adjustment info. I will discuss with my chiropractor. When we go into this surgery we don't know how our bodies will respond. When I was told I reached all the milestones very early, it did not mean much to me because I had nothing to compare it to. On that note, if there are minor complications, we really don't feel the difference. There is Youtube video of Barbara Walters talking about her OHS. She is told there were some issues. He response was, "but I didn't know it."
I can relate to the Living Will stuff. I left a folder on my my counter top with every possible piece of info that could be needed. Things like how to put money in my younger kids lunch accounts all the way to cemetery plot info (thats the jewish mother in me).
It is still sitting where I left it on Dec. 5th, I think I am superstitious about putting it away.
You know you will look back at this so very soon, but the anticipation sucks.

Hi Jeff, I've been wondering "where you have been". I figured you were pretty much in the same place I am. Frankly, right now I am in sort of a fugue state I think. Not exactly afraid, not excited anymore and like you, the reality of Chris's experience sort of slapped me in the face. He appears to be so healthy otherwise, in great shape and I thought, "dang, he was so ready, so calm (I guess) is so young and in great shape". And he had this frightening (at least to me) complication. As you said, I had gotten so accustomed to the great, no complication success stories, well this just made me stop and pause again. I don't think I have had the "big cry" yet. Lots of little ones, lol. I am so glad Mitch is in NYC and could be there in person to support Niki and Chris. I worry about my daughter being alone in Cleveland if I should have problems.
Ok, time to start over. Have only a week before leaving, need to get my mind back to my "peaceful place".

I have been reading every one of your entries and I must tell you, I am sooo proud of you! I must admit, sometimes when I think of you I still see you as the little annoying sudo-brother I grew up with. How did this pain in the neck, refrigerator snooping kid turn into such an intelligent, successful, articulate man??? You impress me so much with your how much you've researched and planned. I have complete confidence that you are in the best possible hands and that you will come through with flying colors. (I just hope the hospital has enough food for you while you're there.)
In all seriousness, you are always in my heart and thoughts and please remember, there are at least 2 cozy bed and breakfasts in Oceanside, NY that are open 24/7 should you need them!

You obviously have to what 'til its all healed. I can't recall who far out you are now. When you are healed and ready, I would ask the chiropractor if he/she does "anteriority" thoracic adjustments. That is done face up and MUCH more comfortable than a face down thoracic adjustments. Also, the vertebrae move much easier this way, with less force. You need a chiropractor who is GOOD at these. Many, I hate to say it, are not proficient in thoracic anteriority adjustments. Also, when you get a good, comfortable thoracic adjustment, it helps the chest too because the ribs adjoin both the spine in the rear and the sternum in the front. Let me know if you have any other questions. Sorry but can't reply March 7 and 8th at least :)
