So, last week, I found myself in a hybrid catheterization laboratory \/ operating suite at the University of Virginia in Charlottesville, Virginia. Dr. Scott Lim, Associate Professor of Pediatrics and Medicine, Division of Cardiology, was performing <a href=\"https:\/\/www.heart-valve-surgery.com\/mitraclip\/\">a MitraClip procedure<\/a>.<\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: center;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone\" src=\"\/Images\/adam-pick-scott-lim.jpg\" alt=\"Dr. Scott Lim &amp; Adam Pick\" width=\"400\" height=\"270\" \/><br \/>\nDr. Scott Lim, M.D. And Me (Adam)<\/p>\n<p>&nbsp;<\/p>\n<p>\u201cWe\u2019ve been investigating the MitraClip therapy in select patients as an alternative, less-invasive therapy <a href=\"https:\/\/www.heart-valve-surgery.com\/mitral-valve-regurgitation-symptoms-leaking.php\">for mitral regurgitation<\/a>,\u201d Dr. Lim shared with me. \u201cBy clipping the mitral leaflets into a double-orifice configuration we can reduce leakage across the valve \u2013 without inducing stenosis.\u201d<\/p>\n<p><!--more--><\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: center;\"><iframe data-src=\"https:\/\/www.youtube.com\/embed\/PWmLU8A9vXk\" loading=\"lazy\" src=\"\" width=\"560\" height=\"315\" frameborder=\"0\" allowfullscreen=\"allowfullscreen\"><\/iframe><\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: left;\">While the MitraClip is commercially available in Europe, the device has yet to receive FDA approval in the United States. Abbott Laboratories, the manufacturer of the MitraClip, submitted results from its EVEREST trial to the FDA in March, 2010. Reports suggest that FDA approval could occur in 2011. Until then, only 20 MitraClip procedures are performed each month in the United States &#8212; under the FDA&#8217;s ongoing access protocol.<\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: center;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone\" src=\"https:\/\/www.heart-valve-surgery.com\/Images\/mitra-clip.jpg\" alt=\"\" width=\"233\" height=\"170\" \/><br \/>\nThe MitraClip By Abbott Laboratories<\/p>\n<p>&nbsp;<\/p>\n<p>Thus far, Dr. Lim has performed over 60 MitraClip procedures. That positions him as one of the most experienced interventional cardiologists with this technology. Only <a href=\"https:\/\/www.heart-valve-surgery.com\/heart-surgery-blog\/2010\/07\/14\/dr-saibal-kar-alfredo-trento-mitraclip-cedars-sinai\/\">Dr. Saibal Kar, at Cedars-Sinai Hospital<\/a> in Los Angeles, has performed more MitraClip procedures.<\/p>\n<p>As I observed Dr. Lim with his medical team and patients, my assumption about this married, father of two, was confirmed\u2026 Dr. Lim is an incredibly nice guy who prefers to be called \u201cScott\u201d not \u201cDr. Lim\u201d.<\/p>\n<p>I also found Dr. Lim to be a straight shooter about the opportunities and constraints of this percutaneous technology.<\/p>\n<p>&nbsp;<\/p>\n<blockquote><p>\u201cWhile the MitraClip appears to be a viable option for mitral regurgitation treatment, I believe we, the physicians, need to ensure that only the right patient gains access to its clinical use at this point in its development. Other patients may be better served by the more traditional, open-heart surgical approaches to treat mitral valve disease.\u201d &#8212; Dr. Scott Lim<\/p><\/blockquote>\n<p>&nbsp;<\/p>\n<p>During the procedure, I continued to learn more about the MitraClip and its unique delivery system. I was intrigued by the way in which Dr. Lim and Dr. Gorav Ailawadi, Assistant Professor of Cardiac Surgery, manipulated the controls of the MitraClip\u2019s delivery system in front of an 8-screen display panel.<\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: center;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone\" src=\"\/Images\/lim-ailawadi-mitraclip.jpg\" alt=\"\" width=\"350\" height=\"394\" \/><br \/>\nDr. Lim and Dr. Ailawadi Adjust The MitraClip<\/p>\n<p>&nbsp;<\/p>\n<p>During the hour-long procedure, Dr. Lim and Dr. Ailawadi shared the controls of the MitraClip and engaged in a lively discussion about positioning the device in the patient\u2019s beating heart. On their 2nd attempt at &#8216;clipping&#8217; the mitral leaflets, real-time echocardiogram results illustrated a significant improvement in reducing the patient\u2019s mitral regurgitation.<\/p>\n<p>\u201cWith this device, we\u2019re able to evaluate results and then adjust if needed,\u201d Dr. Lim said to me. \u201cOn our first attempt, the posterior leaflet wasn\u2019t sitting as it should. So, we repositioned the MitraClip to achieve a better outcome.\u201d<\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: center;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone\" src=\"\/Images\/mitraclip-evaluation.jpg\" alt=\"\" width=\"400\" height=\"266\" \/><br \/>\nDr. Lim and Dr. Ailawadi Evaluate Reduction In Mitral Regurgitation<\/p>\n<p>&nbsp;<\/p>\n<p>According to reports, the MitraClip delivery system (shown below) offers physicians eight different planes of movement to maneuver the clip through the patient\u2019s vascular system and heart.<\/p>\n<p>&nbsp;<\/p>\n<p style=\"text-align: center;\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone\" src=\"\/Images\/mitraclip-delivery-system.jpg\" alt=\"\" width=\"400\" height=\"266\" \/><br \/>\nThe MitraClip Delivery System<\/p>\n<p>&nbsp;<\/p>\n<p>\u201cWe got a good result today,\u201d Dr. Lim shared with me as the case came to a close. \u201cThe patient\u2019s mitral regurgitation decreased from a 3-4 to a 1-2 level.
